Sara Shively, PA-C 

Sara Shively, PA-C, is a Family Practice Physician Assistant who enjoys seeing patients of all ages. She specializes in pediatrics, women’s health, and substance use recovery. 

 Sara is eager to receiving training in medication-assisted treatment at Axis. Her personal experience with substance use within her family has ignited a strong desire to help others struggling with addiction. 

Born in Virginia, Sara graduated from Jefferson College of Health Sciences in 2009. She has been living in Pagosa Springs with her husband and two daughters for over ten years. Sara also enjoys participating in community outreach projects, spending time with her family, camping, boating, and traveling.
