Axis Health System is an integrated healthcare provider. Your records may include medical, mental health, substance-use treatment or dental care. Requests for all information (including progress notes, consent forms, lab results, etc.) can delay processing, can sometimes be much more information than you really need and may include a fee. You or your legal representative have the right to your health records. Find out about your rights and more from the Office of the National Coordinator for Health Information Technology.
