Applying for the Sliding Fee Discount Program

Follow these steps to apply and benefit from our services.

Please complete the application and include proof of income (POI) documents listed below. Bring the application and supporting information to the location where you receive care. Only one application per family/household is permitted.

If you currently do not have a source of income, please inform us. This does not exclude you from the program.

To apply for the Sliding Fee Discount, you must produce family/household proof of income (for anyone listed over the age of 18) to qualify.

Q. What documents are required for the application?

To apply, you need to provide proof of income, identification, and any other documents that support your financial situation.

Approved proof of income can include:

  • Last 30 days of recent pay stubs (must be consecutive)
  • Current filed tax return
  • Current W-2
  • Letter confirming unemployment
  • Current Letter confirming Social Security income
  • Letter from place of employment on company letterhead. The letter must include a contact name and phone number, and the gross amount earned for the time period.

Personal checks and bank statements are NOT accepted.

Q. How long does the application process take?

We can typically review applications immediately. The review process may be delayed if all of the proof of income documents are not included with your application. We strive to process applications as quickly as possible.

Yes, the program is available to both insured and uninsured individuals who meet the income criteria.
