Substance Use Intensive Outpatient Program
Our Substance Use Intensive Outpatient Program is open to join and participate anytime. Treatment is progress-based, and participants engage in this level of case as long as clinically necessary. This service is available in Montrose and Durango, Colorado.
Our Intensive Outpatient Treatment Program is easy to understand. It uses the MATRIX model which includes tested and effective methods like Early Recovery Skills, Relapse Prevention, and Family Education. We also offer social support via the SMART Recovery program. Once you complete the program, you’ll receive further care tailored to your specific needs.
Referral Process
Participants of this program may be referred by any primary care provider, behavioral health provider, or judicial partner.
To refer someone to Axis’ Intensive Outpatient Program

Appropriate clients for this program may meet the following criteria:
- Diagnosed with substance use disorder
- Active or recent substance use within the past month
- Would benefit from daily structure to meet treatment goals
- Meets criteria for intensive outpatient level of care (does not need to be determined prior to calling or making a referral)
Starting Intensive Outpatient
The Intensive Outpatient Treatment Program is a comprehensive approach to help individuals with their mental health and substance use problems. This starts with an initial assessment where various aspects of a person’s medical, substance use and mental health history are examined. This crucial step takes about 1.5 hours and shapes the course of the program.
After joining, the program offers frequent check-ins by professionals at different points (like after the 4th and 8th weeks) to examine the person’s progress. There may also be drug tests to ensure the person is staying accountable and committed to recovery.
At the end of the program, there’s a final evaluation to see the progress made during the program and any changes in substance use. Suggestions are also provided for future treatments.
Throughout the program, a dedicated team consisting of clinicians, family therapists, case managers, and a recovery coach provide support and guidance.
Ongoing Care
Once you finish the Intensive Outpatient Treatment Program, we’ll work with you to figure out the best next steps for your recovery journey. You might start seeing a new therapist to focus on different needs, and we’ll still coordinate with other parties as required.
Recovery can take time – up to 48 weeks or more. During this period, we will provide you with individual therapy, substance use groups, and co-occurring groups to ensure your steady progress. As you move forward in your recovery, the frequency of these services will gradually decrease. Once you’ve met your treatment goals, you can opt for regular check-ins with us to keep up your momentum.
Individual therapy at this stage mainly focuses on recovery and might also address your mental health issues, underlying causes of substance use, and more, using a range of therapeutic methods.
Substance use groups are recommended next, building on the skills you’ve already developed in the program. These groups will provide you the insight and support needed to improve your life and maintain your progress.
Co-occurring groups are also available to those dealing with mental health and substance use problems concurrently. We offer a variety of group options tailored to your unique needs.
Treatment Services
EARLY RECOVERY SKILLS
Early Recovery Skills groups focus on behavior changes to support establishment of sobriety from substances. Groups are structured and routine, and do not regularly include process as part of group.
RELAPSE PREVENTION
Relapse Prevention groups are open, discussion-oriented groups focusing on discovering and preventing the pattern of relapse. Groups explore such topics as guilt and shame, triggers, and motivation.
FAMILY EDUCATION
Family Education groups include significant members of the participant’s family and support circle. These groups provide education to family members on substances, how substance use impacts the family system, and how families can support recovery.
LIFE SKILLS PREVENTION & INTERVENTION
This service supports the establishment of stability and skills for maintaining stability. Life skills groups also serve to engage our clients in other services. Topics include housing, employment, benefits, and harm reduction skills.
SMART RECOVERY
Self-Management and Recovery Training, or SMART Recovery, is a group using tools from effective practices such as cognitive behavioral therapy and motivational interviewing to build motivation, cope with urges, and manage thoughts, behaviors, and feelings.
INDIVIDUAL THERAPY
The relationship between client and therapist is significant to recovery. Through the therapeutic relationship, our clients can develop trust and maintain accountability in their recovery. Our clients participate in individual therapy sessions to encourage deeper reflection into their recovery, and to discover and address underlying reasons for their substance use. Individual therapy may use a variety of trauma-informed therapeutic approaches including cognitive behavioral therapy, solution-focused therapy, cognitive processing therapy, prolonged exposure therapy, or other modalities as appropriate. Following completion of the program, our clients may transfer care to a different therapist for a specialized focus in a related area of treatment.
FAMILY THERAPY
An important aspect of recovery is the client’s support system. Our clients participate in family therapy sessions with significant individuals in their lives to help develop a healthy support system and encourage use of effective relationship skills. This program encourages inclusion of significant persons to help with engagement and support ongoing success after treatment.
MEDICATION ASSISTED TREATMENT (MAT)
Medication Assisted Treatment (MAT) is a groundbreaking approach that combines FDA-approved medications with counseling and behavioral therapies to treat substance use disorders effectively. At Axis, we understand the complexities and challenges of addiction, and MOUD and MAT programs are designed to provide comprehensive, patient-centered care. Using approved medication, our expert team tailors each treatment plan to the individual’s unique needs for an individualized approach to recovery. People can visit Regional Crisis Center – Durango without an appointment, with or without a family member or friend, and receive care. The Axis Health crisis care team also offers mobile crisis services, coming to the location of the person in a crisis. Other services include inpatient substance use withdrawal management, drug screening, 18-hour respite care, and an Acute Treatment Unit (ATU) for people ages 18 and over who need to access short-term, acute outpatient psychiatric care quickly. The ATU allows people to receive acute care while remaining in the community. Regional Crisis Center – Durango also provides intensive and individualized services to help people manage a crisis and stabilize in a secure residential setting.

The Regional Crisis Center – Montrose is for people who need a safe, secure place to recover from a behavioral health or substance use crisis.
